Free Treatment and Training on Asthma, Eczema and Allergies

Eczema Trust Zimbabwe in Partnership with the Ministry of Health will be conducting a free clinic in Norton, Mashonaland west province. the clinic is aimed at empowering communities, raising awareness and providing of training to health care providers on these conditions. the  clinic will be held on the 6th of July 2013 at Katanga clinic in Norton. a team of Specialist Doctors will be treating people for free.


Norton is a fishing town situated about 35Km from the capital city Harare and falls under Mashonaland west Province. majority of the residence in Norton depend on selling fish as their  source of income. most white meat including fish is considered to be cheap compared to other meat. Eczema Trust Zimbabwe has established that there is a lot of myths and misconceptions associated with eating fish as a result people who develop different skin conditions associate their conditions to eating fish. This exercise will be used as documented evidence to establish the truth surrounding that most skin diseases.


In an interview held recently with the Katanga Clinic officials it was also established that they meet almost 80% of both adults and children suffering from skin conditions every moth during their clinic days, this has necessitated the need to conduct the exercise in Norton.


Due to limited resources the exercise will be a one day program and will see Doctors and other health care providers taking part.

Why do some people have a problem with anxiety and others do not?

This is a question almost everyone who experiences anxiety asks themselves at some point or another. Why me?


My understanding of anxiety is that yes, some people seem more susceptible than others but that the key trigger tends to be exhaustion. By exhaustion I mean mental, physical, or emotional exhaustion. (Under physical exhaustion I also include things like diet or substance abuse)


For some it may be exhaustion caused by a hectic life and never taking time to release the stress. People like that often do not notice their stress levels are so high until they get blindsided by a spontaneous panic attack.300x250 Graphics


For others it may be an emotional exhaustion caused by the loss of a loved one or the break up of a long term relationship.


If the anxiety is caused by a traumatic life event it is interesting to note that the person frequently does not experience the anxiety until the event has passed.


You often see people dealing very well with a crisis but then several weeks later when the dust has settled they start to feel the anxiety. It is like they have been in shock and are only now starting to process the experience.


The most important thing to remember about panic attacks or general anxiety is that help is available and it is important to get help sooner rather than later.


I always recommend visiting your doctor first of all to really determine that it is just anxiety you are dealing with and not an underlying physical ailment. Once you are sure that it is anxiety that you dealing with, treat it.


Don’t wait


Burying your head in the sand hoping it will simply be gone next week is not an effective way to treat it. It is totally unnecessary to spend months if not years dealing with something that can be corrected now.

Asthma is often divided into at least two classes that is extrinsic asthma type and intrinsic asthma type. On the other hand others would introduce another class naming it mixed asthma. This can make it a questionable three classes of asthma.

Extrinsic Type of Asthma Extrinsic asthma now also known as allergic asthma is generally brought about by allergens like pollen particles, etc. Intrinsic Type of Asthma Extrinsic asthma is normally brought about by adrenal glands disorder and/or mental disorder such as emotional stress, nerves and anxiety.


A different school of thought further subdivides intrinsic type of asthma into classifications like exercise-induced, occupational type, etc. The category of asthma basically depends on what can be considered to be resulting in the attack. Whether it is external controllable things or something that is not external to the asthma victim the character of the attack are similar. That is to say you will never tell if someone has an intrinsic type of asthma attack or the extrinsic type. Mostly the discomforts are similar. Both Extrinsic and Intrinsic Combined Mixed asthma is a hybrid of intrinsic and extrinsic asthma. Mixed asthma asthma victim react to particular allergies but also are activated by other intrinsic group of stimulants.


It is a two- step issue – When an individual is affected by asthma, inflammation is experienced in the bronchi and it becomes red and swollen. When the air passages get inflamed, the asthma victim becomes extra-sensitive to different things, which may intensify asthma. How Can You Tell If It’s An Attack It is a respiratory disease brought on by allergies; it can be characterized or its presence might be felt by symptoms such as chest constriction, labored breathing and consistent coughing.

Most people affected with allergies start crossing their fingers and refreshing their medication the minute they get one whiff of spring’s bounty of grasses, trees and flowers.


But once cooler weather hits, there’s another segment of the population that prepares itself for Mother Nature’s mischief. The mayhem surprisingly can happen indoors too – as people escape the frigid temperatures outside. Take a look at some of these cold-weather culprits.







COLD-INDUCED URTICARIA








Cold Facts: Some people get rosy cheeks in the cold weather. For others, plummeting temperatures cause a breakout of cold-induced urticaria, also known as “cold-induced hives,”  They’re a reaction to cold weather or cold water and are often itchy – can be as large as a half-dollar, may appear suddenly and may reoccur chronically with exposure to cold.




Remedy: The appearance of cold-weather hives requires a visit to a medical doctor who’ll test for abnormal proteins in the blood, called cryoglobulins, that are activated by exposure to the cold. If cryoglobulins are present, further evaluation is needed to check for underlying causes, including hepatitis C.

Other precautions to take include keeping the body as covered up as possible and taking a nondrowsy antihistamine before going out in the cold or into cold water. If medication doesn’t help, a physician may suggest Periactin, a prescription drug with the active ingredient cyproheptadine, which has side effects such as drowsiness and possible weight gain.





COLD-INDUCED ANGIOEDEMA








Cold Facts: Cold-induced angiodema – swelling in the deeper tissues – is often associated with cold-induced urticaria. Swelling is potentially a more dangerous response because it may precipitate a drop in blood pressure. This, in turn, may lead to light-headedness or more serious symptoms. In contrast with the itching of hives, cold-induced angioedema typically causes burning or pain.




Remedy: If taking a nondrowsy antihistamine before going outdoors doesn’t help, following the pharmaceutical protocol for cold-induced urticaria may provide relief.





RAYNAUD’S PHENOMENON








Cold Facts: In some individuals, exposure of the hands and feet to the cold may lead to severe constriction of the blood vessels. The result is decreased oxygen to those areas, which causes the skin to go white and become painful. As the skin warms up, the affected body parts often turn a rainbow of colors – blue followed by red. When Raynaud’s becomes chronic, the skin, tissue under the skin and even muscles may waste away. Fingernails may become brittle and marked by longitudinal ridges.




Remedy: It’s important to seek additional medical attention to rule out Raynaud’s syndrome, also known as secondary Raynaud’s. Secondary Raynaud’s may be associated with systemic lupus erythematosus (SLE), other immune disorders and even medications such as beta blockers. In extreme cases, the fingertips may be so adversely affected that skin ulcerations or gangrene may result.

Protective clothing, especially for the extremities, is a must. If SLE has been ruled out but the patient still experiences severe symptoms, a visit to a vascular specialist for further evaluation is suggested.





COLD-INDUCED ASTHMA







Cold Facts: For some individuals, simply breathing cold air into the lungs is all it takes to bring on an asthma attack, with shortness of breath and constriction of the airways. Cold dry air is sometimes a more potent trigger than the cold moist variety. Cold winds can also fuel an attack, and typical winter-weather flu and colds can worsen asthma symptoms.






Remedy: To keep your mouth and nose warm and help ensure that you’re breathing warm air, generously cover these body parts with protective outerwear. Carry a rescue inhaler, such as albuterol or levalbuterol, at all times, and use it before exposure to the cold. If necessary, avoid exercising outdoors in frigid temperatures or even in unheated indoor facilities.





VASOMOTOR RHINITIS








Cold Facts: Although it’s technically not an allergy, vasomotor rhinitis – the common runny nose – nevertheless behaves like an allergy. Symptoms of this condition – continuously runny noses, sneezing, nasal congestion and post-nasal drip can all be part of the mix – may suddenly erupt in cold-sensitive people after they do nothing more than set their feet down on a cold floor.




Remedy: Antihistamines typically provide no relief for this condition. However, prescription nasal sprays – including nasal antihistamines, nasal cholinergic inhibitors (ipratropium), or nasal steroids – may provide relief.





DUST MITES








Cold Facts: It’s far more comfy watching snow, sleet and bundled-up neighbors on a plush sofa or pillowed up in bed on a soft mattress. But you may have company. House dust mites, which allergists often refer to as HDM, also like to stay - in upholstery, bedding and linens.



These microscopic critters have quite an appetite, gobbling up skin that humans regularly shed. As they dine, the mites secrete digestive enzymes, especially proteases, which wind up in their fecal matter. These substances – as well as the mites’ external structure – may trigger allergic symptoms that include runny noses, wheezing, congestion, headaches, itching and asthma. Fortunately, blocking the activities of Dermatophagoides farinae and other dust mite brethren is relatively easy.




Remedy: Dust mite covers do a great job keeping these at a safe distance. That’s because the weave of these special fabrics is very tight – typically less than 10 microns – which helps ensure that mites can’t squirm their way in and out of your possessions. As a precaution, wash bedding at least once a week in very hot water. If family members are dust mite-sensitive, keep fabrics in the home to a minimum by eliminating drapes and opting for blinds or other window treatments.





EVERGREEN TREES








Cold Facts: Bringing nature indoors is one way to fuel the winter-holiday spirit. Unfortunately,  decking your home with evergreen trees, wreaths or garlands may also heighten allergies. That’s because evergreens may harbor naturally forming mold on their needles and fruits.




Remedy: Some holiday tree farms put their trees into a “shaker” with an eye towards dislodging the mold. But if your tree still provokes allergic reactions, try an OTC nondrowsy or long acting antihistamine. An artificial tree may be the most trouble-free solution of all.





FABRICS








Cold Facts: With an onslaught of cold weather, most people start insulating by layering their clothing. Trouble is, some winter fabrics, such as wool, contain short and rigid fibers that can provoke skin itching. The people most susceptible are often those who have thinner skin or existing skin conditions, such as eczema.




Remedy: To cut down the scratchiness quotient, make sure the first-layer garment touching your skin is made of 100 percent cotton. Once that cotton layer is in place, consider experimenting with synthetic fabrics, especially garments that athletes wear in the cold weather. Read clothing labels carefully, and stay away from garments that have been coated with resins or other chemicals. And, because dry skin is more susceptible to irritation from fabrics, moisturizing your skin is a must.





FOOD ALLERGIES








Cold Facts: One tasty antidote to cold weather is the variety of food that becomes available as holiday time approaches. However, there are seldom labels of ingredients on the pot-luck offerings from family and friends or the food served at catered events or office parties. For people who are food-sensitive or allergic to certain foods, the season presents the problem of having less control over what’s being served.




Remedy: Good communication is key. Ask if these foods – fruit cakes, eggnog and casseroles are often red flags – contain ingredients on your “do not eat” list, especially nuts. Be vigilant and, if necessary, abstain from eating potentially appetizing but harmful dishes. If you have already been diagnosed with a food allergy, be sure to carry your emergency medication with you at all times.





PETS








Cold Facts: Is there ever too much time with your pets? Yes, if you are holed up with them inside the house after having been absent from your pets for several months, as in the case of a student returning home for a visit from a pet-free dormitory.

Pets typically emit allergenic proteins in their bodily secretions which can end up in the pets’ hair and skin. These proteins then become part of the pets’ dander, which may be an allergy trigger in some people.




Remedy: Don’t allow pets free run of the house and specifically keep them out of the bedroom. Bathe and groom your pets frequently, and use a high-efficiency particulate air filter – where they congregate.

Bronchiectasis


Bronchiectasis is destruction and widening of the large airways.



  • If the condition is present at birth, it is called congenital bronchiectasis.

  • If it develops later in life, it is called acquired bronchiectasis.


Causes


Bronchiectasis is often caused by recurrent inflammation or infection of the airways. It most often begins in childhood as a complication from infection or inhaling a foreign object.


Cystic fibrosis causes about half of all bronchiectasis in the United States. Recurrent, severe lung infections (pneumonia, tuberculosis, fungal infections), abnormal lung defenses, and obstruction of the airways by a foreign body or tumor are some of the risk factors.


The condition can also be caused by routinely breathing in food particles while eating.


Symptoms


Symptoms often develop gradually, and may occur months or years after the event that causes the bronchiectasis.


They may include :



  • Bluish skin color

  • Breath odor

  • Chronic cough with large amounts of foul-smelling sputum

  • Clubbing of fingers

  • Coughing up blood

  • Cough that gets worse when lying on one side

  • Fatigue

  • Paleness

  • Shortness of breath that gets worse with exercise

  • Weight loss

  • Wheezing

Assessment



  1. History or presence of supporting factors

    • Smoking

    • Living or working in areas with severe air pollution

    • History of allergies in the family

    • There is a history of acid in childhood.



  2. History or the presence of trigger factors such exacerbations :

    • Allergen (pollen, dust, skin, pollen or fungal)

    • Emotional Sress

    • Excessive physical activity

    • Air pollution

    • Respiratory tract infections

    • The failure of the recommended treatment program




  3. Physical examination by focusing on the respiratory system include :

    • Assess the frequency and respiratory rhythm

    • Inpeksi color of skin and mucosal color menbran

    • Auscultation of breath sounds

    • Make sure that when patients use accessory muscles when breathing :

      • Lifting the shoulders during breathing

      • retraction abdominal muscles during breathing

      • Respiratory nostril



    • Assess if the symmetrical or asymmetrical chest expansion

    • Assess if the chest pain on breathing

    • Assess cough (whether productive or nonproductive). When you specify the color of sputum productive.

    • Determine if the patient has dispneu or orthopneu

    • Assess the level of consciousness.




Nursing Diagnosis and Intervention



  1. Ineffective airway clearance related to increased production of viscous secretions or secretion.

    Goal :
    Keep the airway patent with breath sounds clean / clear.


    Result Criteria :
    Showed the behavior to improve airway clearance (effective cough, and issued a secret.


    Action Plan :



    • Monitor the frequency of respiration. Note the ratio of inspiration and expiration.

    • Auscultation of breath sounds and record breath sounds.

    • Assess the patient to a comfortable position, height headboard and sat on the back of the bed.

    • Help the abdominal breathing exercise or lip.

    • Observations karakteriktik cough and Auxiliary measures for effectiveness cough efforts.

    • Depth of fluid intake till 3000ml/day appropriate cardiac tolerance and provide a warm and fluid intake between meals in lieu.

    • Give the drug as indicated.



  2. Changes in nutrition less than body requirements related to nausea, vomiting, sputum production, dispneu.

    Goal :
    Improvement in nutritional status and body weight patients


    Result Criteria :
    Patients did not experience further weight loss or maintain weight.


    Plan of action :



    • Monitor input and output every 8 hours, the amount of food consumed and body weight are weighed each week.

    • Create a fun atmosphere, an environment free of odor during mealtimes.

    • Refer patient to a dietitian to monitor food plan that will be consumed.

    • Encourage clients to drink at least 3 liters of fluid per day, if not get an IV.

Can Obesity Cause Depression


Obesity and A depressive disorder
 Occasionally we make the mistake of believing in stereotypes simply due to the fact legends, books, movies, television and other forms of media tell us that they’re true. Santa Claus is a perfect example.


 The Victorian-age poem “A Visit From St. Nicholas” tells us (little kids, in specific) that Santa Claus can be a happy, jolly, nimble, and had a belly that “shook when he laughed like a bowl full of jelly.” Only an obese adult would wonder why Santa is so jolly, and how he manages to slinker down all those tight chimneys.


 Not only is obesity distinctly un-funny, but obese folks can’t wriggle into jeans, significantly much less down chimneys.


 According to the Mayo Clinic, chronic and clinical depressive disorders is “caused by a complex set of physical, psychological and environmental factors. Often, a stressful living event can trigger depressive disorder.”


 Despression symptoms is a considerable sickness that far exceeds feelings of sadness, and causes clinically significant changes in a person’s body, mood, thoughts and behavior. It can affect both guys and females of any age, and is an equal-opportunity destroyer of both loved ones lifestyle as well as the living in the person who is depressed.


 The American Psychiatric Association describes the signs and symptoms of key depressive disorder as:


 Depressed mood most on the day, each day
 Diminished interest in activities once thought pleasurable and interesting
 Sleep disturbance – too significantly, too small
 Appetite disturbance – as well significantly, too little
 Either physical agitation or lack of physical energy
 Excessive fatigue
 Feeling helpless, hopeless, worthless, guilty
 Inability to focus or concentrate
 Thoughts of suicide


 Symptoms trigger significant interference with home, school family members or social lifetime.


 Folks who are obese typically suffer from either chronic (on-going) depressive disorder or episodes of major depression for primarily emotional and social reasons but also when there is a change in physical situation just like acute sickness, menopause or child-bearing, chronic illness (especially pain) and chronic obesity.


 The world today is all about becoming thin and good-looking. Most media-based characters, primary female, are thin too the point of starvation – cases in point, Holly Hunter in Saving Grace and just about everyone on Grey’s Anatomy and Private Practice. Depicted as glamorous, young, sexy, and also the ultimate in fashion sense, these ladies are impossible role models for today’s real females. Still, since major depressive disorder primarily affects females, not living up to these models can trigger extreme reduction of self-esteem and signs or symptoms of acute despression symptoms.


 The National Institutes of Psychological Health report that a depressive disorder can exist alongside other psychiatric complications for instance anxiety and eating disorders. These illnesses may perhaps precede the despression symptoms, cause it, and/or be a consequence of it.


 When depressive disorders strikes males, the self-worth issues are frequently disguised by getting irritable, angry, and by even violent behavior. Thus, despression symptoms may well be tough to recognize in males.


 Even if a man realizes that he is depressed, he may well be less willing than a woman to seek support since our society also negatively stereotypes as “weak” individuals men who are anything but physically and emotionally strong. Clinical depression that’s severe sufficient for the affected individual to look for cure doesn’t go away on its own. Asthma Remedies

Asthma is an ancient Greek word meaning ” panting or short- drawn breath.” It is the most troublesome of the respiratory diseases. The asthma patient gets frequent attacks of breathlessness in between which he is completely normal.


Asthma Symptoms
Patients suffering from asthma appear to be gasping for breath. Actually, they have more trouble exhaling than inhaling because the air passages of the small bronchi become clogged and constricted with mucus, thus making it difficult for the patient to breathe out. All asthmatics have more difficulty at night, especially during sleep. The onset of asthma is either gradual or abrupt. Sudden onsets are often preceded by a spell of coughing which may be associated with itching of the chin, the back of the neck or chest. When the onset is gradual, the attack is usually brought on by respiratory infection. A severe attack causes an increase in heartbeat and respiratory rates and the patient feels restless and fatigued. There may be coughing, tightness in the chest, profuse sweating and vomiting. There may also
be abdominal pain, especially if coughing is severe. Foggy weather aggravates the symptoms. An asthmatic attack begins when the bronchial tubes in the lungs become constricted. The tubes having become narrow, the inhaled air becomes trapped in the tiny air sacs at the end of the tubes, making the release of breath difficult. The wheezing sound identified with asthma is produced by the air being pushed through the narrowed bronchi.


Asthma Causes
Mainly bronchial in its symptoms (Asthma bronchitis), asthma is caused by a variety of factors. For many it is an allergic condition resulting from the reaction of the system to the weather, food, drugs, perfumes and other irritants which vary with different individuals. Allergies to dust are the most common. Some persons are sensitive to the various forms of dust like cotton dust, wheat dust and paper dust, some pollens, animal hair, fungi and insects, especially cockroaches. Foods which generally cause allergic reactions are wheat, eggs, milk, chocolates, beans, potato, pork and beef. For others, asthma may result from the abnormal body chemistry involving the system’s enzymes or a defect in muscular action within the lungs. Quite often, however, asthma is precipitated by a combination of allergic and non- allergic factors including emotional tension, air pollution, infections and hereditary factors. It has been estimated that when both parents have asthma or hay fever, in 75 to 100 per cent cases, the offspring also has allergic reactions.
Asthma Treatments
Modern medical system has not been able to find a cure for this crippling disease. Drugs and vaccines have only limited value in alleviating symptoms. Most of these are habit forming and the dose has to be increased from time to time to give the same amount of relief. The frequent introduction of drugs in the system, while giving only temporary relief, tends to make asthma chronic and incurable. Allergy – which is the immediate cause of asthma – itself is an indication of lowered resistance and internal disharmony caused by faulty eating and bad habits. This is the root cause and the real cure lies in a return to nature.

The natural way treatment of asthma consists of stimulating the functioning of slack excretory organs, adopting appropriate diet patterns to eliminate morbid matter and reconstruct the body, and practicing yogasanas, yogic kriyas and pranayamas to permit proper assimilation of food and to strengthen the lungs, digestive system and circulatory organs. The patient should be given an enema to clean the colon and prevent auto-intoxication. Mud-packs applied to the abdomen will relieve the fermentation caused by undigested food and will promote intestinal peristalsis. Wet packs should be applied to the chest to relieve the congestion of the lungs and strengthen them. The patient should be made to perspire through steam bath, hot foot bath, hot hip bath and sun bath. This will stimulate the skin and relieve congested lungs. The patient should fast for a few days on lemon juice with honey and thereafter resort to a fruit juice diet to nourish the system and eliminate the toxins. Gradually, solid foods can be included. The patient should, however, avoid the common dietic errors. Ideally, his diet should contain a limited quantity of carbohydrates, fats and proteins which are acid-forming foods, and a liberal quantity of alkaline foods consisting of fresh fruits, green vegetables and germinated gram. Foods which tend to produce phelgm such as rice, sugar, lentils and curds as also fried and other difficult- to- digest foods should be avoided. Breakfast may consist of prunes, orange or berries or a few black raisins with honey. Lunch and dinner should consist of a salad of raw vegetables such as cucumber, lettuce, tomato, carrot and beets, one or two lightly cooked green vegetables and wheat bread. The last meal should preferably be taken before sunset or at least two hours before going to bed.
Asthamtics should always eat less than their capacity. They should eat slowly, chewing their food properly. They should drink eight to 10 glasses of water a day, but should avoid taking water or any liquid with meals. Spices, chillies and pickles, too much tea and coffee should also be avoided.
Asthma, particularly when the attack is severe, tends to destroy the appetite. IN such cases, do not force the patient to eat. He should be kept on fast till the attack is over. He should, however, take a cup of warm water every two hours. An enema taken at that time will be very beneficial.
Honey is considered highly beneficial in the treatment of asthma. It is said that if a jug of honey is held under the nose of an asthma patient and he inhales the air that comes into contact with the honey, he starts breathing easier and deeper. The effect lasts for about an hour or so. This is because honey contains a mixture of ‘higher’ alcohols and ethereal oils and the vapors given off by them are soothing and beneficial to the asthma patient. Honey usually brings relief whether the air flowing over it is inhaled or whether it is eaten or taken either in milk or water. It thins out accumulated mucous and helps its elimination from the respiratory passages. It also tones up the pulmonary parenchyma and thereby prevents the production of mucous in future. Some authorities recommend one year old honey for respiratory disease.
Another effective remedy for asthma is garlic. The patient should be given daily garlic cloves boiled in thirty gms of milk as a cure for early stage of asthma. Steaming ginger tea with minced garlic cloves in it, can also help to keep the problem under control and should be taken both in the morning and evening. Turmeric is also regarded as an effective remedy for bronchial asthma. The patient should be given a teaspoonful of turmeric powder with a glass of milk two or three times daily. It acts best when taken on an empty stomach.
During the attack, mustard oil mixed with little camphor should be massaged over the back of the chest. This will loosen up phelgm and ease breathing. The patient should also inhale steam from the boiling water with caraway seeds, known as ajwain in the vernacular. It will dilate the bronchial passage.
The patient should also follow the other laws of nature. Air sun and water are great healing agents. Regular fasting once a week, an occasional enema, breathing exercises, fresh air, dry climate, light exercises and a correct posture go a long way in treating the disease.
The patient should perform yogic kriyas such as jalneti, vamandhouti and yogic asanas such as ekpaduttanasana, yogamudra, sarvangasana, padmasana, bhujangasana, dhanurasana, vakrasana, ardh-matsyendrasan, shalabhasan, paschimottanasana and shavasana. Pranayamas like kapalbhati, anuloma-viloma, ujjayi, surbyabhedana and bhramari are also highly beneficial.
The patient should avoid dusty places, exposure to cold, foods to which he is sensitive, mental worries and tensions. Asthmatic should be made to feel that they are not sick, and with slight adjustments, can live a full life.

Remarkably, I am feeling rather good today. Full day, lots to do, busy, and I was up for it. Enjoying my work. A plan is in action, I don’t have to stew over why now, what’s happening to me. The time for intellectualizing the issue is over, I need a different way in, and that is in the works. Nothing more for me to do, but stay calm and… um, carry on.


Patient, big guy, in pain not only for where he was having surgery, but also the other side – which seemed to be bothering him even worse.


“So, when do we do the other one?”


“Tomorrow!” he booms.


He’s loud and funny, and we like him. He’s a bit difficult, but apologizing enthusiastically. An Extrovert, used to being in charge, but appreciative of people who know what they’re doing. We keep him laughing, he makes us laugh.


As he’s about to be induced, the resident anesthesiologist puts the full O2 mask on him, and he orders her, “GET THAT OFF MY FACE!” She tries to explain as she is getting her drugs going, and he repeats the demand several times. “GET THAT OFF MY FACE!!!”


“No.” I say, and hold it more firmly.


He mumbles it out one more time, quieter, as the drugs take hold.


I have seen many people irritated with that mask, pulled on it, asked for it to be gone, complained of it. But I have never been brusquely ORDERED to remove it, with the clear expectation that he’d be obeyed. Dude, you are not in charge here. On this point, I am.


Once one decides to trust a particular expert, it’s wise to let them lead the dance. Keep a certain reserve, of course, but let the sherpa do the job.


How is COPD Treated?

There is no cure for COPD. Damage to your airways and lungs cannot be reversed, but there are things you can do to feel better. COPD symptoms can be managed and damage to the lungs can be slowed. The most important thing you can do to slow the progress of the disease is to quit smoking. Your doctor may recommend treatments to relieve your symptoms and there are things you can do to live better with the disease. The goals of COPD treatment are to:


• Relieve symptoms with no or minimal side effects of treatment
• Slow the progress of the disease
• Improve the ability to stay active and exercise
• Prevent any complications from the disease
• Improve health overall


Treatment for each person is different and is based on whether symptoms are mild, moderate or severe. There are a variety of treatments that can help reduce symptoms and manage complications, including:


• Bronchodilators – medications that work by relaxing the muscles around your airways, opening them and making it easier to breathe. They can last either 4-6 hours or 12 hours, depending on the type. Most are inhaled directly into the lungs with the use of an inhaler.
• Inhaled steroids – used for some people with moderate or severe COPD to reduce the inflammation or swelling in the airways.
• Vaccines – it is recommended that you get a pneumococcal vaccine to prevent pneumonia and an annual flu shot to avoid any breathing complications.
• Pulmonary or lung rehabilitation (rehab) – helps you stay active despite your COPD. It includes exercise training, nutrition advice and education about managing the disease. A custom pulmonary rehab program is managed by a multidisciplinary team (doctors, nurses, respiratory therapists, exercise specialists, dietitians).
• Oxygen therapy – for severe COPD and low levels of oxygen in the blood. Using extra oxygen can help you perform tasks or activities with less shortness of breath, protect
the heart and other organs from damage, improve the quality and duration of your sleep, improve your alertness during the day and help you live longer.
• Surgery – is usually done for patients who have severe symptoms, have not improved with medications and have a hard time breathing most of the time. There are two types of surgery for COPD: a bullectomy, which removes a large air sac that may compress a good lung, or a lung transplant.


The goal of COPD treatment is to prevent or minimize the progression of the disease. During a disaster or emergency situation it may be difficult to follow all treatment recommendations. Stopping treatments may increase the frequency or severity of breathing difficulties. You may also become overly tired and eventually require hospitalization. You should also call your doctor if you have sudden chest tightness, more coughing, a change in your sputum or fever.


This could signal a lung infection. It is important that all treatment recommendations be followed to the best of your ability in order to maintain lung function, prevent further lung damage and maintain your quality of life.

Little Devil"s Tower, Custer State Park 2009



 Little Devil’s Tower trail in Custer State Park is the best hike that Wayne and I have been on so far. When we were in Custer State Park in 2009 it was recommended to us by the cashier at the gift shop when we told her we were looking for a hike that would give us a view of the Cathedral Spires. She told us it was a “fairly easy” hike and the best view of the spires so we breezed right by the information on the board at the trailhead and started walking. The first 1/2 mile meandered through some trees alongside a nice brook with a very gradual incline. I thought, hey, no problem! This is great! But after that first half mile you take the #4 trail to the left and things change drastically with  the trail becoming so steep and so rocky that we were slowed down quite a bit. However, we were undeterred, thinking this had to be temporary, because the trail was supposed to be “fairly easy”, right?


 






 It did level out a bit, and we came to this big crack between the rocks. I stopped to catch my breath and take a few pictures and thought the worst was behind us and we’d be seeing those spires at any moment. I was struggling to breathe because I was dealing with some as yet undiagnosed allergy-related asthma. This trip opened my eyes and when we got home I went to the doctor and now am the proud owner of an inhaler!  During our hike we noticed that park officials had set out pheromone traps in an effort to stop the spread of the pine beetle.  There were piles of cut down trees everywhere that the beetles had been lured to. Check to make sure this trail is open before heading out, because apparently they closed it after we left to continue their campaign against this pest.




 But after passing through the crevice…the steep climb began again, and we followed the blue diamonds or paint on the trees and kept going up, and up, and up! We were starting to get a little worried because the sky was clouding up and we didn’t want to be on that trail if it started to rain, but we pushed on!


Until finally we came around a bend and saw something that looked vaguely peak-like! But we still weren’t done, so we made that final push and with quivering legs we clambered up to the top of Little Devil’s Tower, still following blue paint, this time right on the rock itself.





It was worth the 1.5 mile climb, most of it nearly vertical. We had only passed one other couple during our hike and we had the top of the world all to ourselves. We spent a good 1/2 hour just resting and enjoying the views which included not only the Cathedral Spires but everything you could see for miles in every direction. I only wish the light had been better in order to capture how absolutely gorgeous it was up there!





 






We were up so high that a tour helicopter came up right alongside us and we all waved to each other. This helicopter I got the picture of came later and didn’t come right by us but went straight to the Cathedral Spires instead. I thought it was a pretty neat shot with the rain clouds though. We didn’t get an opportunity to hike this trail again when we were at Custer briefly last year, but if we ever get over there again I’m definitely going to head up with my camera gear at a better time of day and get some photos! Anybody in the area should make this hike a priority, it’s moderately strenuous, but only 3 miles round trip and worth every step!
